Affirmative Statement

Utilization Management (UM) Affirmative Statement regarding incentives will be signed by all UM decision-makers annually. Elite Care Health Organization does not give financial incentives for UM decision-makers and does not encourage denials of coverage or service. Decision-makers include medical directors, UM and case management managers, Quality Improvement Committee members, licensed staff and management personnel. Providers may freely communicate with patients about their treatment, regardless of benefit coverage limitations.

The Utilization Management (UM) staff is available for callers with questions about approvals, denials, or UM criteria eight hours per day, Monday-Friday. Patients can call (888)975-3246.

Practitioners can obtain the following upon request: UM policies, procedures, and criteria used to authorize, modify, or deny healthcare services to providers, members or member designated representatives. Criteria and policies are distributed by mail, fax, e-mail, provider manual, provider intranet site or on its website, if information is available online.  Criteria can be mailed to practitioners who do not have fax, email or internet access. Providers can call 88-975-3246 ext 114 for request Monday- Friday 8:00am-5:00pm.
